Admissions

Admissions and referrals

Families, guardians, caseworkers and partner agencies can contact CONQER to ask about a placement for a child or young adult ages 5 to 21. Here is what to expect.

Who can refer

Parents and guardians, state and county caseworkers, schools, healthcare providers and partner agencies are all welcome to reach out. If you are unsure whether CONQER is the right fit, ask us. We are happy to talk it through.

The admissions process

  • 1. Reach out: email info@conqer.org with a short description of the child's needs. Please do not include sensitive medical details in a first email.
  • 2. Conversation: our admissions team follows up to learn more and answer your questions.
  • 3. Assessment: we review the child's needs, history and goals with the family and referring professionals.
  • 4. Planning: if CONQER is a good fit, we build a personalized care plan together before the child arrives.

What to have ready

It helps to know the child's age, the kind of support they need, and who is involved in their care. Our team will tell you if any additional paperwork is needed.

Safe. Seen. Supported.
